Crystal Maxwell, MD, MBA

Family Medicine

Biography

Dr. Crystal Maxwell is from Foxworth, MS where she graduated with honors from West Marion High School as Valedictorian in 1999. She proceeded to Spelman College and graduated Magna Cum Laude College with a B.S. in Biology in 2003. She graduated from the University of Tennessee College of Medicine with a medical doctorate in 2007 and went on to specialize in Family Medicine at the University of Tennessee Family Medicine Residency Program in Jackson, TN in 2010. Dr. Maxwell completed the Physician Executive MBA program at Auburn University in 2016.

After completing her residency training in 2010, Dr. Maxwell joined Sandhills Medical Foundation at its Jefferson, SC location as a Family Physician and she continues to serve in this position. Sandhills Medical Foundation is a federally qualified community health center that serves people of all backgrounds with specific assistance provided to those with financial disadvantages. She’s responsible for the total care of her patients. Dr. Maxwell manages both chronic diseases such as diabetes, hypertension, and emphysema and acute problems such as respiratory and gastrointestinal infections. In 2012, as a result of her hard work, dedication to serving the mission of the organization, team approach to patient care, and leadership abilities, Dr. Crystal Maxwell was promoted to Medical Director. She is responsible for the management of the clinical aspects of all 7 Sandhills Medical Foundation locations and she has also served as the clinical representative to the South Carolina Primary Health Care Association Board of Directors since 2018.

​Education, health promotion, and community service have been important to Crystal from an early age. She volunteered at the nursing home and the local Special Olympics during high school. In college, she was a health peer educator and led forums on various health topics such as STDs, nutrition/exercise, and chronic diseases. She also helped raise money by participating in several walks for HIV, heart disease, and breast cancer awareness. During medical school, Crystal spent part of a summer volunteering in the Save a Million Lives program and provided HIV/AIDS education in Ghana. Upon moving to the Carolinas, she became a member of the Women’s Ministry and Healthcare Ministry at Freedom Temple Ministries in Rock Hill, SC. She serves as a healthcare ministry coordinator and while serving she provides health education using the theme “How’s your LIGHT? | How’s your Living In God’s Holy Temple, which is a part of her independent company, Crystal A. Smith Maxwell, MD, LLC for health education and motivational speaking. She has been an advocate for heart health awareness and raising funds for the American Heart Association yearly.

Though Dr. Maxwell has enjoyed giving back while managing a great career, her greatest joy comes from being both a wife and mother. She’s been married to her husband, Carleton, for 12 years and has a beautiful and vibrant 8-year-old daughter, Caitlyn, who keeps her very active.
